Composition:
Isosorbide dinitrate 10mg
Uses / Indication:
Prevention and treatment of angina pectoris (chest pain caused by coronary artery disease)
Heart failure (often combined with other medicines like hydralazine)
Pulmonary hypertension (in some cases)
Post–myocardial infarction management (after heart attack in selected patients)
Direction of Use:
Usually taken 2–3 times daily or as prescribed by a healthcare provider
Often with a nitrate-free interval (10–14 hours) each day to prevent tolerance
Side Effects:
Common;
Headache (very common with nitrates)
Dizziness or light-headedness
Flushing
Low blood pressure
Fast heartbeat (tachycardia)
Nausea
Less Common / Serious
Severe hypotension
Syncope (fainting)
Reflex tachycardia
Methemoglobinemia (rare with high doses)
Warnings/Interactions:
Use cautiously in patients with:
Severe hypotension
Hypovolemia (low blood volume)
Recent head injury or increased intracranial pressure
Severe anaemia
Hypertrophic cardiomyopathy
Right ventricular infarction
Other warnings:
Tolerance may develop with continuous use.
Must maintain a nitrate-free interval daily.
May cause postural hypotension (dizziness when standing).
Interactions:
Major (Avoid completely)
Sildenafil
Tadalafil
Vardenafil
Combination can cause life-threatening hypotension.
Other interactions
Antihypertensives → additive blood pressure lowering
Alcohol → increased dizziness/hypotension
